Services

The following services are provided for free to all patients who visit the SLO Noor Foundation Free Clinic:

Basic Primary Care

  • Patient Education
  • Disease Prevention & Screening
  • Physical Exams
  • Women’s Health, including Gynecological care
  • Men’s Health
  • Laboratory Testing
  • Radiology
  • Specialty Referrals
  • Screening to help you qualify for Medi-Cal, CenCal, or similar free public insurance programs
  • Nutritional support
  • Behavior Consultation
  • Physical Therapy

Medical Services

Designed to meet the essential need that has arisen in our community, medical services are the cornerstone of The SLO Noor Foundation. Our clinic provides a “medical home” to adults with acute and chronic illnesses needing routine medical supervision and medications.

Services provided include general and acute medical care, gynecological, STD diagnosis and treatment, diabetes treatment and hypertension treatment. These services are offered on a mixed appointment and walk-in schedule. Individuals needing specialty care such as pulmonology, cardiology, rheumatology, podiatry or oncology and other sub-specialties in future, may receive such care in the clinic or in the offices of specialists who volunteer with the SLO Noor Foundation Free Clinic. As with all the free clinic services, most necessary tests are free of charge.

Case Management/Social Services

The case management department at The SLO Noor Foundation assists our patients in obtaining needed services in the community. Case managers work with clients to help them identify their needs and goals. Clients are then provided with referrals and information that is aimed at meeting those needs to assist them in achieving their goals. Our services are provided within a short-term framework that encourages the empowerment of clients to ensure them the opportunity to control their own future.

Referrals are commonly in the areas of:

  • Housing
  • Transportation
  • Substance abuse issues
  • Emergency food assistance
  • Skilled Nursing or Assisted Living Placement Services

Women’s Health

The SLO Noor Foundation Free Clinic is especially committed to providing ongoing support of women’s health through regular screening and follow-up appointments. Staff members assist patients in accessing needed care for breast and other types of cancer. Services are provided at the clinic during regularly scheduled hours.
