Case Management/Social Services
The case management department at The SLO Noor Foundation assists our patients in obtaining needed services in the community. Case managers work with clients to help them identify their needs and goals. Clients are then provided with referrals and information that is aimed at meeting those needs to assist them in achieving their goals. Our services are provided within a short-term framework that encourages the empowerment of clients to ensure them the opportunity to control their own future.

Referrals are commonly in the areas of:

  • Housing
  • Transportation
  • Substance abuse issues
  • Emergency food assistance
  • Skilled Nursing or Assisted Living Placement Services
Disease Prevention & Screening
Preventing disease generally begins with your diet and exercise. Eating healthy foods and being physically active play major roles in preventing disease. These behaviors reduce your risk of diseases such as diabetes, heart disease, and a wide variety of cancers. They also improve your mental and emotional health.

If you are affected by disease, early detection is important and can even save your life. Health care professionals, such as those at the Noor Clinic, keep you informed about how often we should have tests or lab work to detect possible signs of illness or disease. These screenings can allow you to get preventive care before symptoms of the disease even appear.
Screenings and preventive care differ according to the particular disease. For example, not smoking (or quitting smoking) is basic to preventing lung cancer. To help prevent diabetes, heart disease and many cancers, we can avoid becoming overweight or obese.

Your health care providers can advise us about how often we should have tests such as mammograms (to look for signs of breast cancer), or a blood test to check for unhealthy levels of cholesterol (this test is called a "lipid panel").

As scientific research reveals new knowledge about screenings and diseases, the recommendations about how often (and at what ages) you should have such tests may change. That is one of the benefits of seeing a health care provider periodically: The medical staff will let you know which tests or lab work you should have done. Follow-up appointments provide the opportunity for you to learn about the meaning of the results for maintaining our health or treating symptoms of disease.

As patients, it is important for you to learn more about your health and preventing disease so that we may be partners with the doctors and nurses that care for us. The better we understand the causes of illness and ways to improve our health and fight disease, the better are our chances for leading healthy, long lives. Heart Awareness Classes
Heart disease is America’s number one killer of men and women, and half of those who suffer a heart attack don’t even know they are at risk. SLO Noor teams with French Hospital’s HeartAware program, which incorporates Lipid Panel testing to help determine your heart disease risk factors and to help guide you through steps to lead a healthier lifestyle. Your SLO Noor physician will help you with this program.
Laboratory Testing
Lab tests administered at the SLO Noor clinic serve a variety of purposes. They monitor your body’s systems and check for signs of disease. They also alert us to conditions that may develop into disease, so that preventive changes can be made. The SLO Noor medical staff will inform you about which tests you need and their purpose.”

The SLO Noor clinic offers many lab tests, including: Basic Metabolic Panel, Comprehensive Metabolic Panel, EKG, Lipid Panel, Liver Panel, Urine Dip, and many others. Your SLO Noor physician will advise you and administer the needed tests.
Hepatitis C Testing
If preliminary lab tests administered at the SLO Noor clinic give an indication of possible Hepatitis C, SLO Noor will order further testing from Sierra Vista Regional Medical Center.
Mammograms
A mammogram is a low-dose x-ray that allows doctors called radiologists to look for changes in breast tissue. A mammogram can often find or detect breast cancer early, when it’s small and even before a lump can be felt. This is when it’s easiest to treat. SLO Noor patients are referred to French Hospital for mammograms.
Nutritional Classes for Diabetes and Diabetes Prevention
SLO Noor partners with Dignity Health, which offers a Diabetes Empowerment Education Program, an evidence-based education program for people with diabetes or prediabetes. The program encourages lifestyle changes while educating patients about diabetes and the way it affects quality of life.
Nutritional Support
SLO Noor partners with Dignity Health, which offers a Healthy for Life Wellness Program. The program’s classes include information on food labels, portion size, eating different colored vegetables, and fats and sugars. There are also interactive food demonstrations, fun physical activities, heart and stroke education, and screenings for cardiac risk, blood glucose, stroke risk, and blood pressure.
Patient Education
Each and every SLO Noor patient receives education directly related to their health challenges, conditions, and illnesses. Always ask your SLO Noor physician if you have any questions. Your health is their priority.
Physical Exams
All adults should visit their health care provider from time to time, even if they feel healthy. The purpose of these visits is to: screen for diseases, assess future medical risks, encourage a healthy lifestyle, update vaccinations, and maintain a relationship with a health care provider.
Physical Therapy
Physical therapy is a health care specialty involved with evaluating, diagnosing, and treating disorders of the musculoskeletal system. The ultimate goal is to restore maximal functional independence to each individual client, utilizing exercise, heat, cold, massage, and mild electrical stimulation.

SLO Noor partners with San Luis Sports Therapy, which provides free therapy for our patients. Your SLO Noor physician can refer you for a physical therapy appointment.
Radiology
Radiology Associates partners with SLO Noor to provide free x-rays, ultrasounds, MRI’s, CT scans, and other needed services for patients.” **Referrals for Medi-Cal, CenCal, or similar free public insurance programs – “The SLO Noor staff will work with you to determine if you are eligible for any government or other public health insurance programs.

If necessary, SLO Noor staff can refer patients to our collaborating medical partners and providers. These include: Central Coast Cardiology, Dr. Debra Truell OB/GYN, The Colonoscopy Center, San Luis Obispo Sports Therapy, Alliance for Pharmaceutical Access, French Hospital Medical Center, Sierra Vista Regional Medical Center, Radiology Associates of SLO, Planned Parenthood, Marian Regional Medical Center, Vision Services Plan, People’s Self-Help Housing, Hearst Cancer Center, Central Coast Counseling, Transitions Mental Health, and Central Coast Pathology.
