Eligibility Requirements
SLO Noor patients must not have any type of health insurance including: private health insurance, government provided health coverage, MediCal, CenCal or any other type of health insurance.
SLO Noor patients may be asked to prove they are not eligible for Medi-cal.
Household income must not be more than 200% of the current year's Federal Poverty Level for all household members. View current poverty guidelines here.
SLO Noor patients must be between the ages of 18 to 65.

Preventing disease generally begins with diet and exercise. Eating healthy foods and being physically active play major roles in preventing disease and help reduce the risk of diabetes, heart disease, and a wide variety of cancers. They also improve your mental and emotional health.
Since early detection is important and can even save your life, SLO Noor provides various screenings and testing for diseases. The medical staff will let you know which tests or lab work you should have done and schedule follow-up appointments to provide the opportunity for you to learn about the meaning of the results. The better we understand the causes of illness and ways to improve our health and fight disease, the better are our chances for leading healthy, long lives.

Lab tests administered or ordered at the SLO Noor clinic monitor your body’s systems, check for signs of disease, alert us to conditions that may develop into disease, and provide an opportunity for preventive lifestyle changes to be made. The SLO Noor medical staff will inform you about which tests you need and their purpose. Testing may include: Hepatitis C Screening and Testing, Basic Metabolic Panel, Comprehensive Metabolic Panel, EKG, Lipid Panel, Liver Panel, Urine Dip, and many others.

A mammogram is a low-dose x-ray that allows doctors called radiologists to look for changes in breast tissue. A mammogram can often find or detect breast cancer early when it’s small and even before a lump can be felt. This is when it’s easiest to treat. SLO Noor patients may be referred for mammograms.

SLO Noor partners with Radiology Associates to provide x-rays, ultrasounds, MRI’s, CT scans, and other needed services for patients. These services are offered at no charge to patients.

SLO Noor offers onsite nutrition education and lifestyle coaching for people with diabetes or prediabetes. The program includes educating patients about diabetes, food recommendations and substitutions, and activity goals.

All adults should visit their health care provider from time to time, even if they feel healthy. The purpose of these visits is to: screen for diseases, assess future medical risks, encourage a healthy lifestyle, update vaccinations, and maintain a regular relationship with a health care provider.

Physical therapy is a health care specialty involved with evaluating, diagnosing, and treating disorders of the musculoskeletal system. The ultimate goal is to restore maximal functional independence to each individual client, utilizing exercise, heat, cold, massage, and mild electrical stimulation.
SLO Noor’s physical therapy partners provide free therapy to patients. Your SLO Noor physician can refer you for a physical therapy appointment, if needed.

The SLO Noor Foundation assists our patients in obtaining needed services in the community, and providing them with referrals and information. Referrals are commonly in the areas of:
• Free or highly-discounted prescription medication assistance
• Housing
• Transportation
• Substance abuse issues
• Food assistance
• Mental Health Issues
• Governmental insurance enrollment assistance -
Each and every SLO Noor patient receives education directly related to their health challenges, conditions, and illnesses. Always ask your SLO Noor physician if you have any questions. Your health is their priority.

If necessary, SLO Noor staff can refer patients to our collaborating medical partners and providers.
Cancellation and No Show Policy
Please read this carefully, as we discharge patients from the Clinic for not keeping appointments.
Please be on time or early for your appointments.
You will be discharged from the Clinic and not receive further care if you:
No-show for 2 appointments. A cancellation less than 24 hours ahead of your appointment will beconsidered a no-show.
Cancel or are late for more than 2 appointments.
